Participant forms and guidance

Thank you for your interest in the 2017 Aon Long Term Care Professional Liability and General Liability Actuarial Analysis. Your confidential participation helps us to produce reliable estimates of liability costs for the industry. As a participant, you will receive an advance, electronic copy of the report prior to public release in early November 2017.

To participate in the 2017 benchmark report, please do the following:

1. Download the Consent Form, Data Call Instructions, and the Long Term Care Report Data Call template.
2. Complete the Consent Form and email it, along with your data in the Data Call template or similar format, to: [email protected]. All submissions must be received by July 31. Please include your organization’s name in the file name.
