Participant forms and guidance

Thank you for your interest in the Aon 2019 General and Professional Liability Benchmark for Long Term Care Providers. Your confidential participation helps us to produce reliable estimates of liability costs for the industry. As a participant, you will receive an advance, electronic copy of the report prior to its public release in October 2019.

To participate in the 2019 benchmark report, please do the following:

1. Download the Consent Form, Data Call Instructions, and the Data Call template.


2. Complete the Consent Form and email it, along with your data in the Data Call template or similar format, to [email protected]. All submissions must be received by Monday, April 15.


3. Please include your organization’s name in the file name.
